Austin Bradford Hill

Austin Bradford Hill (1897–1991) was an English epidemiologist and statistician who helped establish the modern randomized controlled trial and proposed considerations for assessing causation in observational research.

Randomized controlled trial: A study that assigns participants to interventions by chance to estimate their effects. Hill helped demonstrate how random allocation could make treatment comparisons more reliable.
Bradford Hill criteria: Nine considerations proposed for assessing whether an observed association may be causal. Hill set out these considerations in 1965, including temporality, strength, and biological plausibility.
London School of Hygiene and Tropical Medicine: A London institution for research and education in public health and tropical medicine. Hill held its chair of medical statistics before becoming professor of medical statistics and epidemiology.
Observational study: A study in which researchers observe exposures and outcomes without assigning interventions. Hill’s causal considerations addressed evidence from observed associations, unlike randomized treatment assignment.
Streptomycin trial: A 1948 British Medical Research Council study comparing streptomycin with bed rest for pulmonary tuberculosis. Hill directed this landmark controlled trial, which used randomized allocation and an explicit protocol.
Temporality: The requirement that a cause precede its effect in time. Hill treated temporal order as indispensable when judging whether an association could be causal.
Medical Research Council Streptomycin in Tuberculosis Trials Committee: The committee that organized the British randomized trial of streptomycin for tuberculosis. Hill’s work with the committee helped turn random allocation into a practical trial method.
Nonrandomized trial: An intervention study in which participants are assigned to groups without random allocation. It can test treatments but is more vulnerable than Hill’s randomized design to allocation bias.
Medical Research Council: A United Kingdom organization that funds and conducts biomedical research. Hill worked within the council, whose trial of streptomycin became a model for clinical research.
Confounding: Distortion of an exposure–outcome association by a third variable related to both. An apparent causal relationship can arise because the observed association is confounded.
