Delayed puberty

Delayed puberty is the later-than-expected onset or unusually slow progression of the physical changes of sexual maturation. It may reflect normal variation, chronic illness, or disrupted hormone production.

Hypothalamic–pituitary–gonadal axis: The hormone system linking the hypothalamus, pituitary gland, and gonads to regulate reproduction and sexual development. Delayed activation or signaling along this pathway can postpone pubertal changes.
Constitutional delay of growth and puberty: A common pattern of late growth and pubertal development, often followed by spontaneous progression. This frequent explanation is a variation in timing rather than a permanent failure of puberty.
Tanner scale: A five-stage clinical scale describing physical progression of breast, genital, and pubic-hair development. It provides a standardized way to document whether puberty has begun and how quickly it advances.
Precocious puberty: The onset of pubertal development earlier than the usual age range. It is the timing opposite of delayed puberty and may involve similar hormone pathways.
Gonadotropin-releasing hormone: A hypothalamic hormone released in pulses that stimulates pituitary secretion of luteinizing hormone and follicle-stimulating hormone. Insufficient pulsatile release can prevent the hormonal cascade that starts puberty.
Hypogonadotropic hypogonadism: Reduced gonadal function caused by inadequate hypothalamic or pituitary stimulation. It delays puberty because gonads receive too little hormonal stimulation.
Bone age: An estimate of skeletal maturity, commonly derived from an X-ray of the hand and wrist. A delayed bone age often accompanies constitutional delay and helps interpret growth potential.
Normal puberty: The usual sequence of physical and hormonal changes leading to sexual maturation. Comparing expected onset and progression clarifies what counts as a clinically significant delay.
Luteinizing hormone: A pituitary hormone that stimulates sex-hormone production and reproductive functions in the gonads. Its blood concentration helps distinguish central signaling problems from gonadal dysfunction.
Hypergonadotropic hypogonadism: Reduced gonadal function accompanied by elevated pituitary gonadotropins because the gonads respond inadequately. High gonadotropins point toward gonadal dysfunction rather than delayed central signaling.
