Knowra Derealization Derealization Derealization is a dissociative experience in which the external world feels unreal, distant, dreamlike, or perceptually altered, while the person generally recognizes that this feeling is subjective.
Depersonalization : A dissociative experience in which a person feels detached from their body, thoughts, or sense of self. It is the closely related experience of unreality directed inward rather than toward the external world.
Predictive processing : A theory that perception arises through the brain's combination of sensory signals and predictions about their causes. It offers a framework for considering how altered expectations or sensory weighting might change felt reality.
Clinical assessment : The structured process of gathering symptoms, history, functioning, and other evidence to guide a clinical formulation. Assessment establishes the experience's pattern, distress, triggers, and possible alternative explanations.
Acute stress reaction : A short-term response to an overwhelming event that can include emotional, cognitive, and physical symptoms. Brief unreality experiences can occur during or after intense stress without establishing a chronic disorder.
Dissociation : A disruption in the usual integration of consciousness, memory, identity, emotion, perception, or bodily experience. Derealization is one form of this broader class of experiences.

Migraine aura : A set of reversible neurological symptoms that can precede or accompany a migraine headache. Unusual sensory changes may be mistaken for derealization, making their timing and character clinically relevant.
Depersonalization-derealization disorder : A mental disorder marked by persistent or recurrent depersonalization, derealization, or both, causing significant distress or impairment. This diagnosis applies when derealization is persistent or recurrent and significantly disrupts life.
Threat response : The coordinated bodily and psychological reactions that prepare an organism to respond to danger. Acute stress can coincide with derealization, though no single threat-response mechanism explains every episode.
