Goodpasture syndrome

Goodpasture syndrome is an autoimmune disease in which antibodies attack basement membranes in the kidneys and lungs, causing rapidly progressive glomerulonephritis and pulmonary bleeding.

Anti-glomerular basement membrane disease: An autoimmune disease in which antibodies target the glomerular basement membrane, sometimes also injuring lung alveoli. Goodpasture syndrome is the pulmonary-renal presentation of this antibody-mediated disease.
Rapidly progressive glomerulonephritis: A syndrome of quickly worsening kidney function with crescent formation in glomeruli. It is the characteristic renal manifestation of Goodpasture syndrome.
ANCA-associated vasculitis: A group of small-vessel inflammatory diseases associated with antineutrophil cytoplasmic antibodies. It can cause lung bleeding and glomerulonephritis, but targets vessels rather than basement-membrane collagen.
Ernest Goodpasture: An American pathologist who described pulmonary hemorrhage and glomerulonephritis in a 1919 report. His early case description gave the syndrome its name, before its autoimmune cause was known.
Type IV collagen: A collagen family forming structural networks in basement membranes throughout the body. The targeted antigen is a domain of type IV collagen in kidney and lung basement membranes.
Diffuse alveolar hemorrhage: Bleeding into multiple lung alveoli, often causing breathlessness, anemia, and coughing blood. It is the pulmonary manifestation that can accompany the kidney disease.
Granulomatosis with polyangiitis: A small-vessel vasculitis that commonly affects the respiratory tract and kidneys. Its pulmonary-renal pattern can mimic Goodpasture syndrome despite distinct pathology.
Anti-GBM disease history: The development of scientific understanding of antibody-mediated basement-membrane injury. Its history links Goodpasture’s clinical observations to later identification of the responsible antibodies.
Anti-GBM antibodies: Autoantibodies that bind the glomerular basement membrane, usually targeting type IV collagen. These antibodies drive the kidney and lung injury characteristic of the syndrome.
Anti-GBM disease diagnosis: Diagnosis combining circulating anti-GBM antibody testing, kidney findings, and tissue examination. Serology and biopsy help distinguish this syndrome from other causes of pulmonary-renal disease.
