Institutionalization

Institutionalization is the placement of people in residential institutions for care, treatment, education, or custody. It can restrict autonomy and participation in community life, especially when stays are prolonged or alternatives are limited.

Institutional syndrome: A pattern of psychological and social difficulties associated with prolonged residence in an institution. Long stays can weaken initiative and everyday skills, though the pattern is not inevitable.
Community-based care: Support and services delivered in a person's home or community rather than in a residential institution. It offers an alternative to institutional placement while aiming to preserve community ties.
Civil commitment: Legal authorization to confine or require treatment for a person without a criminal conviction, under defined criteria. It can provide the legal pathway for involuntary placement in a psychiatric institution.
Moral treatment: A nineteenth-century approach to mental illness emphasizing humane care, structured routines, and therapeutic surroundings. Its ideals shaped early asylums, even as those institutions later became overcrowded and custodial.
Right to community living: The claim that people with disabilities have a right to live in the community with access to necessary support. It frames community residence as a matter of rights rather than merely a service preference.
Deinstitutionalization: The reduction or closure of long-term residential institutions, often alongside shifts toward community-based services. It reverses reliance on institutional residence but requires adequate support outside institutions.
Foster care: A system in which children live temporarily with approved caregivers who are not their parents. It is a residential placement for children, but usually in family homes rather than institutions.
Guardianship: A legal arrangement granting a guardian authority to make decisions for a person found unable to make some decisions independently. Guardianship can influence who controls residential and treatment decisions.
Workhouse: A residential institution where people in poverty received relief in exchange for work under restrictive conditions. Workhouses joined assistance to confinement and discipline, a lasting model of institutional control.
Supported decision-making: An approach that helps people make their own decisions instead of transferring decision-making authority to a substitute. It challenges the assumption that institutional placement requires others to decide a person's living arrangements.
