Loop diuretic

A drug that inhibits the sodium-potassium-chloride cotransporter in the kidney’s thick ascending limb, increasing salt and water excretion in urine.

Sodium-potassium-chloride cotransporter 2: A membrane protein that reabsorbs sodium, potassium, and chloride from tubular fluid in the thick ascending limb. Loop diuretics inhibit this transporter to reduce salt reabsorption.
Furosemide: A loop diuretic used to treat edema and hypertension. It is a widely used example of the drug class.
Diuresis: An increase in urine production and excretion. Loop diuretics produce diuresis by reducing renal salt reabsorption.
Thiazide diuretic: A diuretic that inhibits sodium-chloride transport in the distal convoluted tubule. It acts farther along the nephron and generally produces less potent diuresis.
Nephron: The kidney’s microscopic functional unit, consisting of a tubule and associated blood vessels. Loop diuretic action occurs in one specific segment of the nephron.
Thick ascending limb: The nephron segment that reabsorbs sodium, potassium, and chloride while remaining largely impermeable to water. This is the kidney site where loop diuretics act.
Bumetanide: A potent loop diuretic used to treat edema associated with heart, kidney, or liver disease. It offers another clinical option within the class.
Hypokalemia: An abnormally low concentration of potassium in the blood. Increased distal sodium delivery can promote urinary potassium loss during loop-diuretic treatment.
Potassium-sparing diuretic: A diuretic that reduces sodium reabsorption in the collecting system while limiting potassium loss. It contrasts with loop diuretics, which can increase potassium excretion.
Renal medulla: The inner region of the kidney, containing structures that help concentrate urine. The thick ascending limb helps create the medullary gradient weakened by loop diuretics.
