Methyldopa

Methyldopa is a centrally acting antihypertensive drug that lowers blood pressure by reducing sympathetic nervous system activity. It is used for hypertension, including during pregnancy.

Alpha-methylnorepinephrine: A neurotransmitter formed from methyldopa that activates central alpha-2 adrenergic receptors. Methyldopa is converted into this active molecule, which reduces sympathetic signaling.
Hypertension in pregnancy: Elevated blood pressure occurring before, during, or after pregnancy. Methyldopa has a long history of use to manage chronic hypertension during pregnancy.
Labetalol: An antihypertensive drug that blocks alpha-1 and beta adrenergic receptors. It is another commonly used option for hypertension during pregnancy, with a different receptor action.
Orthostatic hypotension: A sustained blood-pressure fall after standing that can cause dizziness or fainting. Reduced sympathetic compensation can make postural blood-pressure drops more likely.
Alpha-2 adrenergic receptor: An inhibitory adrenergic receptor that responds to norepinephrine and related agonists. Activation of central alpha-2 receptors mediates methyldopa’s reduction of sympathetic outflow.
Chronic hypertension: High blood pressure present before pregnancy or diagnosed before 20 weeks of gestation. Methyldopa is one option for treating this condition during pregnancy.
Nifedipine: A calcium-channel blocker used to treat hypertension and angina. It is an alternative pregnancy-compatible antihypertensive with a different mechanism.
Sedation: A state of reduced alertness caused by a drug or other influence. Central nervous system effects make drowsiness a common limitation of methyldopa.
Sympathetic nervous system: The autonomic nervous system division that prepares the body for action and regulates cardiovascular function. Methyldopa lowers blood pressure by suppressing this system’s activity.
Preeclampsia: A pregnancy-specific disorder involving new hypertension and organ dysfunction, usually after 20 weeks of gestation. Methyldopa may control blood pressure, but it does not treat the underlying disorder.
