Osteopetrosis

Osteopetrosis is a group of inherited disorders in which defective osteoclast-mediated bone resorption causes unusually dense, brittle bones. Severe forms can compress nerves and replace marrow space, impairing blood-cell production.

Osteoclast: A multinucleated cell that breaks down bone tissue during skeletal remodeling. Osteopetrosis arises when osteoclast number or function is insufficient for normal bone resorption.
Bone marrow failure: A condition in which marrow cannot produce enough blood cells to meet the body's needs. Reduced marrow space in severe osteopetrosis can cause anemia, infections, and bleeding.
Osteoporosis: A skeletal disorder characterized by reduced bone mass and increased fracture risk. Osteoporosis reduces bone density, whereas osteopetrosis increases it while still leaving bone fragile.
Autosomal dominant osteopetrosis: A usually milder inherited form of osteopetrosis caused by a pathogenic variant in one gene copy. This form often presents later and is frequently associated with CLCN7 variants.
Genotype–phenotype correlation: The relationship between genetic variants and the observable features or severity of a condition. The same broad diagnosis spans different courses, and a variant does not always predict severity precisely.
RANK–RANKL–OPG pathway: A signaling system that regulates osteoclast formation and activity through interactions among RANK, RANKL, and osteoprotegerin. Disruption of this pathway can prevent osteoclast development and produce osteopetrosis.
Cranial nerve palsy: Weakness or loss of function in muscles or senses served by a cranial nerve. Abnormal skull thickening can narrow nerve passages and impair vision, hearing, or facial movement.
Osteomalacia: A disorder of adults in which defective mineralization softens newly formed bone. Osteomalacia reflects inadequate mineralization, unlike the excessive retained bone in osteopetrosis.
Autosomal recessive osteopetrosis: A form of osteopetrosis that occurs when pathogenic variants affect both copies of a gene. Recessive disease includes severe infantile forms with early marrow and neurologic complications.
Osteoclast transplantation: An experimental approach that supplies functional osteoclasts to restore bone resorption. It explores whether targeted cell replacement could avoid some limitations of whole-marrow transplantation.
