Phenacetin

Phenacetin is a synthetic analgesic and antipyretic formerly used in medicines. Its long-term use was linked to kidney damage and cancer risks, leading to withdrawal in many countries.

Paracetamol: A widely used analgesic and antipyretic also known as acetaminophen. The body converts phenacetin into this active pain-relieving metabolite.
Analgesic nephropathy: Chronic kidney damage associated with prolonged use of certain analgesics. Phenacetin-containing mixtures were strongly associated with this kidney disorder.
Harmon Northrop Morse: An American chemist who synthesized phenacetin in 1878. Morse first prepared phenacetin while investigating derivatives of coal-tar chemicals.
Ibuprofen: A nonsteroidal anti-inflammatory drug used to treat pain, fever, and inflammation. It is a widely used alternative for pain and fever that does not share phenacetin’s history of analgesic nephropathy.
Cytochrome P450: A large enzyme family that oxidizes drugs and other compounds. These enzymes contribute to phenacetin metabolism and the formation of reactive metabolites.
Papillary necrosis: Death of tissue in the renal papillae, the tips of structures that drain urine within the kidney. This characteristic lesion can occur in analgesic nephropathy linked to phenacetin.
Coal-tar medicine: Medicines developed from chemicals obtained during coal-tar processing. Phenacetin emerged from the nineteenth-century search for useful drugs among coal-tar derivatives.
Metamizole: An analgesic and antipyretic medicine whose availability varies by country. Its differing safety concerns and national restrictions offer a comparison with phenacetin’s withdrawal.
Drug metabolism: The chemical transformation of substances by living organisms, especially enzymes in the liver. Phenacetin’s effects depend substantially on how the body transforms it.
Urothelial carcinoma: A cancer arising from the lining of the urinary tract. Long-term phenacetin use was associated with increased risk of cancers of the renal pelvis and urinary tract.
