Phenytoin

Phenytoin is an anticonvulsant medication used to prevent and treat seizures. It limits repetitive neuronal firing by stabilizing voltage-gated sodium channels in their inactivated state.

Voltage-gated sodium channel: A membrane protein that opens and closes to let sodium ions cross cells in response to changes in voltage. Phenytoin binds preferentially to inactivated channels, reducing repeated electrical firing.
Focal seizure: A seizure that begins in networks confined to one cerebral hemisphere. Phenytoin can be used to control focal-onset seizures.
Carbamazepine: An anticonvulsant and mood-stabilizing drug that also blocks voltage-gated sodium channels. It treats focal seizures but has different interactions, adverse effects, and monitoring needs.
Gingival enlargement: An increase in gum tissue that can occur as an adverse effect of certain medications. Chronic phenytoin use can cause gingival overgrowth, worsened by poor oral hygiene.
Heinrich Biltz: A German chemist who synthesized hydantoin compounds in the early twentieth century. His chemical work produced the hydantoin scaffold from which phenytoin was developed.
Use-dependent block: A drug-blocking pattern in which inhibition strengthens as a target is activated more often. Frequently firing neurons accumulate more phenytoin-mediated sodium-channel block.
Status epilepticus: A prolonged seizure or repeated seizures without recovery, requiring urgent treatment. Intravenous phenytoin or fosphenytoin may follow initial benzodiazepine treatment.
Valproate: An anticonvulsant that affects GABA signaling and several neuronal ion channels. It offers broader seizure coverage but carries distinct metabolic and reproductive risks.
Phenytoin-induced osteopathy: Bone disease associated with long-term phenytoin use and altered vitamin D metabolism. Enzyme induction can reduce vitamin D availability and contribute to bone loss.
Hydantoin: A five-membered nitrogen-containing chemical ring that forms the core of a family of compounds. Phenytoin is the anticonvulsant member of this chemical family.
