Pralidoxime

Pralidoxime is an oxime antidote that can reactivate acetylcholinesterase inhibited by organophosphates, provided the inhibited enzyme has not undergone aging.

Acetylcholinesterase: An enzyme that terminates cholinergic signaling by breaking down acetylcholine. Pralidoxime restores this enzyme after organophosphate inhibition.
Nerve agent poisoning: Poisoning caused by highly toxic organophosphorus compounds that inhibit acetylcholinesterase. Pralidoxime is used as part of treatment for exposure to nerve agents.
Obidoxime: A bis-pyridinium oxime used as an antidote for organophosphate poisoning in some countries. It is another acetylcholinesterase reactivator with different pharmacology and availability.
Cholinergic system: The network of neurons and signaling pathways that use acetylcholine as a neurotransmitter. Organophosphate toxicity and pralidoxime treatment both act within this signaling system.
Blood–brain barrier: A selective interface that regulates movement of substances from blood into the central nervous system. Limited central nervous system penetration may leave some effects insufficiently treated.
Organophosphate poisoning: Toxic exposure to compounds that inhibit acetylcholinesterase and disrupt cholinergic signaling. This is the poisoning mechanism pralidoxime is intended to counter.
Pesticide poisoning: Illness caused by exposure to pesticides, including organophosphate insecticides. Some pesticide poisonings involve organophosphates for which pralidoxime may be considered.
HI-6: A bis-pyridinium oxime developed as an antidote for poisoning by certain nerve agents. Its effectiveness varies by agent and can differ from pralidoxime's.
Neuromuscular junction: The synapse where a motor neuron communicates with a skeletal muscle fiber. Acetylcholinesterase inhibition at this junction can cause weakness and paralysis.
Organophosphate structure–activity relationship: The relationship between an organophosphate's molecular structure and its biological effects, including enzyme inhibition and aging. Structural differences help explain why pralidoxime effectiveness varies among compounds.
