Tramadol

Tramadol is a centrally acting analgesic used to relieve moderate to moderately severe pain. It combines weak opioid activity with inhibition of serotonin and norepinephrine reuptake.

Mu-opioid receptor: A cell-surface receptor activated by endogenous opioids and opioid drugs to reduce pain signaling. Tramadol and its active metabolite provide analgesia partly through this receptor.
Acute pain: Pain that begins suddenly and usually improves as an underlying injury or illness heals. Tramadol may be prescribed for short-term relief when pain warrants an opioid analgesic.
Codeine: An opioid analgesic whose pain-relieving effect depends partly on conversion to morphine by CYP2D6. Like tramadol, its effects vary with CYP2D6 activity, but it lacks tramadol’s serotonin reuptake inhibition.
Opioid use disorder: A medical condition involving a problematic pattern of opioid use that causes significant impairment or distress. Repeated tramadol use can lead to opioid-related compulsive use and harms.
Pharmacokinetics: The study of how a drug is absorbed, distributed, metabolized, and eliminated by the body. Tramadol’s metabolism and clearance influence its duration, exposure, and variability.
O-desmethyltramadol: The active metabolite formed from tramadol primarily by the liver enzyme CYP2D6. It binds mu-opioid receptors more strongly than tramadol itself.
Chronic pain: Pain that persists or recurs for longer than the usual period of tissue healing. Tramadol is sometimes used when persistent pain has not responded adequately to other treatments.
Morphine: A potent opioid analgesic that acts primarily through mu-opioid receptors. It provides a comparison with an opioid whose analgesic action is not based on tramadol’s mixed mechanism.
Physical dependence: A state in which the body adapts to a drug and withdrawal symptoms follow abrupt reduction or stopping. Regular tramadol use can produce dependence, even when taken as prescribed.
Pharmacogenomics: The study of how genetic variation affects responses to medicines. CYP2D6 variants help explain differences in tramadol’s opioid effects and adverse risks.
