VACTERL association

VACTERL association is the nonrandom co-occurrence of congenital vertebral, anal, cardiac, tracheoesophageal, renal, and limb abnormalities, usually without a single identified cause. Diagnosis generally requires several characteristic features and exclusion of alternative explanations.

Vertebral anomaly: A congenital or acquired structural variation in one or more vertebrae. Vertebral segmentation or formation defects are one of the association’s defining feature groups.
Mesoderm: The embryonic germ layer that gives rise to many connective tissues, muscles, blood, and parts of the urogenital system. Several affected structures develop from mesoderm, but this does not establish one mesodermal cause.
VACTERL association diagnostic criteria: Clinical rules used to identify VACTERL association from characteristic congenital anomalies and exclude alternative diagnoses. Diagnosis typically requires multiple feature groups; exact thresholds and exclusions vary across clinical practice.
David Smith: An American pediatrician and medical geneticist known for developing dysmorphology and describing congenital syndromes. Smith and colleagues reported the recurring pattern of associated congenital anomalies in 1973.
Imperforate anus: A congenital anorectal malformation in which the anal opening is absent or incorrectly positioned. Anorectal malformations are a defining feature group, though their forms vary.
Embryonic development: The process by which an embryo forms and organizes tissues and organs. The affected organs develop during early embryogenesis, when disruption can produce multiple congenital anomalies.
Esophageal atresia: A congenital interruption of the esophagus that prevents normal passage from the mouth to the stomach. Its presence often prompts evaluation for associated tracheoesophageal and other VACTERL features.

Congenital heart defect: A structural abnormality of the heart or great vessels present at birth. Heart defects, including ventricular septal defects, contribute to the cardiac feature group.
