Wolfram syndrome

Wolfram syndrome is a rare inherited disorder marked by diabetes mellitus, optic nerve atrophy, and progressive neurological damage. It is also called DIDMOAD, after its characteristic features.

WFS1: A gene encoding wolframin, a protein involved in endoplasmic-reticulum function and cellular calcium regulation. Most Wolfram syndrome cases result from disease-causing variants in this gene.
Diabetes mellitus: A group of disorders in which blood glucose remains abnormally high because insulin production or action is impaired. Insulin-dependent diabetes is often the first major manifestation of Wolfram syndrome.
Optic nerve: The bundle of nerve fibers that carries visual information from the retina to the brain. Its progressive degeneration causes the optic atrophy characteristic of the syndrome.
Wolfram-like syndrome: A disorder resembling Wolfram syndrome clinically but arising from different genetic or disease mechanisms. Similar symptoms can occur without the usual molecular causes of Wolfram syndrome.
Donald J. Wolfram: An American physician who described the syndrome now bearing his name with R. P. Wagener in 1938. His clinical report established the recognizable association of juvenile diabetes and optic atrophy.
Wolframin: A protein encoded by WFS1 that helps regulate endoplasmic-reticulum homeostasis and calcium levels. Loss of wolframin function links WFS1 variants to cellular stress in affected tissues.
Optic atrophy: Degeneration of optic nerve fibers that causes impaired visual function. Progressive optic nerve atrophy gives the syndrome one of its defining features.
Neurogenic bladder: Bladder dysfunction caused by impaired nervous-system control of bladder storage or emptying. Autonomic and central nervous-system damage can produce urinary problems in affected people.
Mitochondrial diabetes: Diabetes caused by mitochondrial DNA variants, sometimes accompanied by hearing loss and other systemic features. It can combine diabetes and hearing impairment but differs in its genetic basis and typical pattern.
Wolfram syndrome type 1: The WFS1-associated form of Wolfram syndrome, accounting for most cases. This genetic subtype explains the majority of classic presentations.
