Carbamazepine

Carbamazepine is an anticonvulsant medication used to treat epilepsy, trigeminal neuralgia, and bipolar disorder. It reduces repetitive neuronal firing chiefly by blocking voltage-gated sodium channels.

Voltage-gated sodium channel: A membrane protein that opens in response to voltage changes and allows sodium ions to cross cell membranes. Carbamazepine blocks these channels, limiting repeated electrical discharges in neurons.
Focal seizure: A seizure that begins in networks on one side of the brain. Carbamazepine is a standard treatment option for many focal-onset seizures.
Oxcarbazepine: An anticonvulsant structurally related to carbamazepine and used for focal seizures and nerve pain. It has similar uses but generally causes fewer enzyme-mediated drug interactions.
CYP3A4: A liver and intestinal enzyme that metabolizes many medications and other compounds. CYP3A4 converts carbamazepine to its active epoxide metabolite.
Use-dependent block: A drug-blocking pattern in which inhibition strengthens as an ion channel is activated more frequently. Carbamazepine preferentially suppresses rapidly firing neurons through this channel-blocking behavior.
Trigeminal neuralgia: A chronic pain disorder causing brief, severe attacks along the trigeminal nerve. Carbamazepine is a first-line medication for this characteristic nerve pain.
Lamotrigine: An anticonvulsant and mood stabilizer that inhibits voltage-gated sodium channels and affects glutamate release. It is another option for epilepsy and bipolar maintenance, with a different adverse-effect profile.
Enzyme induction: Increased production or activity of drug-metabolizing enzymes after exposure to an inducing substance. Carbamazepine induces several enzymes, lowering concentrations of many co-administered drugs.
Neuronal action potential: A rapid, transient change in a neuron's membrane voltage that carries electrical signals. Repeated action potentials are the electrical events carbamazepine helps restrain.
Bipolar disorder: A mental disorder characterized by episodes of mania or hypomania and often depression. Carbamazepine is used as a mood stabilizer, particularly for manic episodes.
