Clozapine

Clozapine is an atypical antipsychotic medication used mainly for schizophrenia that has not responded adequately to other treatments. Its use requires monitoring for rare but serious blood disorders and other adverse effects.

Dopamine hypothesis of schizophrenia: A family of theories linking altered dopamine signaling to schizophrenia symptoms. Clozapine affects dopamine receptors, but its efficacy cannot be reduced to simple dopamine blockade.
Treatment-resistant schizophrenia: Schizophrenia that has not responded adequately to appropriate trials of antipsychotic medication. This is the principal indication that distinguishes clozapine from routine antipsychotic treatment.
Olanzapine: An atypical antipsychotic used to treat schizophrenia and bipolar disorder. It shares several receptor effects with clozapine but does not have the same established role in treatment resistance.
Agranulocytosis: A severe loss of neutrophils that sharply increases susceptibility to infection. Clozapine can cause this rare blood disorder, making blood-count monitoring essential.
Serotonin receptor: A membrane protein that binds serotonin and triggers cellular signaling. Clozapine acts at several serotonin receptor subtypes alongside its dopamine effects.
Suicide prevention: Interventions intended to reduce the risk of suicide and suicidal behavior. Clozapine is indicated for reducing recurrent suicidal behavior in some people with schizophrenia or schizoaffective disorder.
Risperidone: An atypical antipsychotic used for schizophrenia and other psychiatric conditions. It is a common alternative before clozapine, with different risks for movement symptoms and prolactin elevation.
Neutropenia: An abnormally low number of neutrophils, a type of infection-fighting white blood cell. Clozapine-associated neutropenia is the main blood safety concern tracked by monitoring.
Dopamine receptor: A cell-surface receptor activated by the neurotransmitter dopamine. Clozapine binds dopamine receptors, including D2, with a profile unlike many older antipsychotics.
Schizoaffective disorder: A mental disorder involving psychotic symptoms and substantial mood episodes. Clozapine may be used when psychosis or recurrent suicidal behavior persists despite other treatment.
